Two well-tolerated compounds map onto this problem from different angles. Polaprezinc (zinc L-carnosine) is a mucosa-adherent chelate that acts topically at the site of injury, with several small radiotherapy and transplant studies showing reduced mucositis severity and better tolerability. L-glutamine is a metabolic substrate for epithelial renewal, with a more mixed evidence base — clinically promising in several RCTs, though larger meta-analyses disagree on effect size.
